Henry Ford Health System Registered Nurse Case Manager – Inpatient Case Management – Day Shift in Detroit, Michigan

Title: Registered Nurse (RN) Case Manager

Department: Inpatient Case Management

Status: Full Time (40 Hours / Week)

Shift: Days (6am to 6pm); Every 5thWeekend

Location: Detroit

What to expect in this role:

You’ll assess patients’ needs upon admission and throughout their hospital stay, develop and coordinate individualized care plans, partner with the entire healthcare team to ensure all aspects of care are addressed, serve as a patient advocate, and provide education and available resources information. Additionally, you’ll monitor the utilization of healthcare resources to optimize patient outcomes and minimize costs, facilitate timely and safe discharge plans and be heavily involved in the patients’ transition to the next level of care.

· After 6 months, you’d have the option to work weekends and holidays remotely with equipment provided.

· The Case Management department has recently redesigned into a dyad Social Work/RN Case Manager model where UR is remote, allowing staff to excel in their chosen profession.

EDUCATION/EXPERIENCE REQUIRED:

· Have graduated from an accredited BSN program.

· Possess RN licensure in the state of Michigan.

· Possess a minimum of three (3) years of acute care/inpatient nursing experience.

· Inpatient Case Management and/or discharge planning and care coordination experience strongly preferred.

· Can communicate effectively with patients, families, and hospital personnel, have strong critical thinking and creative problem solving skills and excel at planning.

· Are self-motivated and enjoy working in a fast-paced environment.
